This role has a starting salary of £50,047 per annum, based on a 36 hour working week.
This is an 18-month fixed term / secondment opportunity.

We are excited to be hiring a new Communities and Prevention Manager to join our fantastic Communities and Prevention team. The team is based in Adults, Wellbeing and Health Partnership and fosters community-led innovation including national vanguards such as Green Social Prescribing and Individual Placement and Support in Primary Care. This role will work in a strengths-based way alongside communities to build community capacity and help them thrive.

About the Team

The Communities and Prevention Team is a supportive and inclusive group of 15 staff. We are ambitious about transforming health and social care, so that prevention is at the core of our local offer. Using evidence as our foundation, we work in a creative, strengths-based way to pioneer new models of delivery which achieve better outcomes for residents.

About the Role 
Our ambition is for Surrey people to feel able to contribute to community life, to live healthy, active and fulfilling lives, and make good choices about their wellbeing.
We are looking for a dynamic and inspirational person who can work in a flexible way with communities, staff and partners. The ideal candidate will have experience of innovation and co-design, as well as strong communication, problem-solving and partnership skills.
Health and social care are facing significant increases in the volume and acuity of people's need at a time of financial constraint. Prevention is increasingly recognised as the driver of long-term change. The Care Act 2014 and CQC assurance frameworks promote a strengths-based approach to prevention. 

In this context, this role will support community participation in the delivery of health and social care. The role, based in Public Health, will work collaboratively with partners, especially the voluntary sector, to develop a range of community resources that enable residents to improve their wellbeing, resilience and independence.

We are looking for someone who is tenacious and creative. You will build a high-performing team and will work pro-actively to co-design and co-produce initiatives that support Surrey's 2030 Vision within the context of health and social care. You will show innovation and perseverance to bring about systemic change, and create an environment whereby individuals can be diverted towards informal community-based services that prevent, reduce or delay care and support needs from developing. Our ambition is to embed a model of social care that grows local resources and encourages behaviours which benefit the individual, improve the outcomes they achieve and enables the Council to deliver services within the resources available.

This role will: 
  • Lead a team to deliver excellent outcomes 
  • Interpret the changing political and social context and identify opportunities for innovation, including liaising with strategy and policy colleagues, and using insight and intelligence to inform decisions and review activity 
  • Scope and deliver projects that will grow community skills and strengths-based opportunities for residents. 
  • Establish new models of delivery which will allow adult social care in Surrey to deliver better outcomes for residents at less cost, including scoping new approaches to supporting and shaping support opportunities provided by the voluntary, community and faith sector 
  • Work alongside health and care staff to embed cultural change and best practice, including embedding strengths-based approaches and behaviour change techniques.
  • Work alongside residents to support community capacity building initiatives, especially amongst communities who experience health inequality.
You will need the willingness and ability to travel around the county to meet the demands of this role.

  • Experience of networked leadership, building collaborative teams of staff from different organisations and teams to work together and achieve a common goal. 
  • Experience of delivering projects to time and budget which made an evidence-able difference to beneficiaries
  • Experience of innovation and working creatively and with perseverance to pioneer new approaches.
