As a child becomes a young adult, the services they work with may need to change to meet their needs. From fourteen years old plans should be made in the Education Health and Care (EHC) plan review meetings, including what support the young person will continue to need when they leave school.

Adult Social Care
Adult Social Care aims to empower people with complex or severe special educational needs or disabilities, by providing them with services that increase their wellbeing and independence.
Children and Adolescent Mental Health Service (CAMHS)
CAMHS offer a range of services to children and young people who are experiencing mental health problems.

Children with Disabilities Team
This team is part of Surrey's Children's Services. They are responsible for assessing the needs of families and providing practical support to children with complex needs and their carers.
CYP Haven
The CYP Haven is a safe space for children and young people aged ten to 18 to go to talk about worries and mental health in a confidential, friendly and supportive environment.
There are Haven's in Guildford, Epsom, Staines and Redhill.
Job Centre Plus
Disability Employment Advisors at the Job Centre are responsible for supporting people with SEND to get a job.
Surrey Youth Voice
Surrey Youth Voice helps shape the services and support available to young people in Surrey. They work with Surrey County Council to create opportunities for young people to share their views, take part in groups and projects and influence real change in their communities. There are also paid roles that help young people build skills and confidence. Surrey Youth Voice supports young people to speak up and makes sure their voices are heard.
There are several participation groups, including ATLAS, which is for young people with additional needs and disabilities. No diagnosis is required - only an interest in improving support and services for young people with additional needs and disabilities. ATLAS offers a safe and supportive space for young people to share their experiences. Members meet regularly to discuss what is working well and what could be improved. They work with professionals across health, education and social care to help shape better services. ATLAS also leads awareness projects and ableism and inclusion, making sure that the voices of young people with additional needs and disabilities are listened to and respected in decisions that affect them.

Special Education Needs and Disabilities (SEND) Team
Education Health Care and Needs assessment officers are allocated to a child or young person when a request to assess has been submitted. If an Education Health and Care (EHC) plan is issued you are transferred to an Education Health and Care plan coordination officer. They will provide advice and support to both school and parents and can attend review meetings when more guidance is needed.
Adolescent Service
Adolescent Service (formerly Targeted Youth Service) can provide support for young people (12 plus) and their families on a variety of issues that can be hard to cope with during adolescence. This includes helping to return to education or training, reducing anti-social behaviour, and improving access to community activities.

Transition Team
The Transition Team provide the link between Children's Social Services and Adult Social Care. The young person's care needs are the focus for the Transition Team.
