Joint Health and Social Care Dementia Strategy for Surrey 2022 – 2027
Surrey Heartlands, Frimley Health and Care, Surrey and Borders Partnership and Surrey County Council have collaborated on a new strategy to support those with dementia, their family and unpaid carers. This strategy sets out the collective ambitions we want to achieve across Surrey to improve the dementia care pathway. In developing this strategy, we have worked with organisations that support people with dementia, their staff, the local voluntary, community and faith sector and other partners.
We wish for all people living with dementia and their unpaid carers to live in dementia-friendly communities where they feel empowered and know where to go to seek information, advice and help. In addition, we aspire that people have access to the care and support that enables them to live well at home for as long as possible, and to die with dignity.
