Providers with concerns about quality of care should report it to our Quality Assurance team. Examples of low level care quality issues that may not necessarily meet the safeguarding threshold based on the 2 documents mentioned below include in relation to:
- pressure ulcers
- falls
- medication errors
- interpersonal incidents among adults

- Care Quality Commission (CQC)
- the appropriate Integrated Care Board (ICB) Quality team
