Step 3: Complete any supplementary forms (primary, infant and junior)


Summary of this step

You should complete any supplementary forms required by the schools that you have named on your application form.

You must do this by the date specified by the school.

Where a supplementary form is needed, you must complete and return this to the address stated on the form, along with any supporting evidence.


Some Surrey schools need you to submit a completed supplementary form in addition to the application form. It provides the school with extra information relevant to its admissions criteria, to help the school when considering your application.

A number of schools, including all community and voluntary controlled schools, give priority to children whose parent is a member of staff at the school. Please see the admission arrangements for each school to see how member of staff is defined and the conditions that may be attached. If you wish to apply for priority under this criterion you will be required to complete the supplementary information form for the school and return it to the address stipulated on the form.

Some schools, including all community and voluntary controlled schools, also require you to complete a supplementary information form if you wish to apply on the basis of exceptional medical or social need. This form will help you provide all the information that is needed for a request to be considered on the basis of medical or social need.

Your application will still be considered if you do not provide any required supplementary information form, but it may mean your child receives lower priority for a place if insufficient evidence is provided.

Find out the closing date for the return of each supplementary form and the address it should be returned to by checking the form or by contacting the school.

If you are applying for schools outside Surrey, contact the school or relevant local authority directly to check what supplementary forms may be needed.

Submitting a completed supplementary form is not a valid application unless you also name the school as a preference on the local authority's application.
