Step 5: Complete any supplementary forms

Summary of this step

You complete any supplementary forms in addition to the application. Check our directory of Surrey schools to find out if a school asks for a supplementary form and download the form.

Where a supplementary form is needed, you must complete and return this to the address stated on the form.

Some Surrey schools need you to submit a completed supplementary information form in addition to the application form. It provides the school with extra information relevant to its admissions criteria, to help the school when considering your application.

A number of schools, including all community and voluntary controlled schools, give priority to children whose parent is a member of staff at the school. Please see the admission arrangements for each school to see how member of staff is defined and the conditions that may be attached. If you wish to apply for priority under this criterion you will be required to complete the supplementary information form for the school and return it to the address stipulated on the form.

Some schools, including all community and voluntary controlled schools, also ask you to complete a supplementary information form if you wish to apply on the basis of exceptional medical or social need. The supplementary information form will help you provide all the information that is needed for a request to be considered on the basis of medical or social need.

Your application will still be considered if you don't provide the supplementary information form, but it may mean your child receives lower priority for a place if sufficient evidence is not provided.

Check our directory of Surrey schools to find out if a school asks for a supplementary form and download a copy.

Submitting a completed supplementary form is not a valid application unless you also name the school as a preference on the CMA or SMA application form.
