Use this form to request to have payments by BACS or tell us of a change to your bank details. This is only available if your account is eight numbers or less.

Parents should use this form to tell you if they want to change the number of hours of Funded Early Education for Two year olds (FEET) they claim for their child. Settings must keep these forms.

Custom Dates Form 2024 to 2025 (PDF)
Please use this form if your term dates fall outside Surrey term-time dates in new EYES system. Please return completed form to Funded Early Education Team.
