It will help to make decisions and make sure that what the person wants is respected.

Talking today can help people plan for their future with confidence.

The Victoria and Stuart toolkit was made with people with learning disabilities, families, learning disability support staff, and healthcare professionals.

It includes ways to support staff with end of life care planning with people with learning disabilities.
There are 3 guides:

- understanding end of life care planning

- how to talk about dying

- a toolkit for supporting people with a learning disability

The guides have information in different formats including video and easy read.

You can find information about end of life care for people of all ages across Surrey through Connect to Support Surrey.

End of life care training is also provided by Phyllis Tuckwell Hospice Care which is funded by the NHS.
