NHS five-year plans

Health and social care services are changing – working closer together to support residents' care needs.

Surrey is made up of three geographical ‘health’ areas:

  • Surrey Heartlands – covering Guildford, Waverley North West Surrey and Surrey Downs clinical commissioning groups.
  • Frimley Health and Care – covering Surrey Heath, North East Hampshire and Farnham CCG (and some areas outside the county).
  • Sussex and East Surrey – covering East Surrey (and some areas outside the county) 

In each of these areas a Sustainability and Transformation Plan (STP) has been developed. This sets out how health and social care services need to change over the next five years to achieve the right care – both now and in decades to come.
