Mental Health Transition Survey: deadline - ongoing

We have relaunched the Transition survey which aims to hear young people's views and experiences, so that we can feed the information back to the relevant services for them to implement positive change. We hope that this will help other young people in the future.

We have created a Transition Questionnaire to gain feedback from young people and hear their views. This is an ongoing project therefore; this is no time limit for young people to complete this survey. Please note that this is for all young people that have transitioned out a service, they do not necessarily have to be 18 and transitioned into an adult mental health service. Additionally, you can find more resources available here.

Post 16 Transition Survey – deadline: 30 January 2021

Do you have a Special Educational Need or Disability? Are you age 14+?

If the above is YOU, we need your help! We are currently asking young people who are due to or going through a change of education, employment or training to fill in the survey.

We would like to know if you are getting enough support with your transition. We would like to know what other support you think may be helpful. Your response will be fed back to Surrey County Council to help improve young people's experiences. Your personal information will be anonymous.

The Big Survey 2020

The Big Survey is an annual questionnaire for looked after children and care leavers. Completing this survey gives children and young people the opportunity to give feedback on services and have their voice heard.

Each year we use the responses received to improve our service and practice throughout Surrey. This survey has been distributed to Surrey's children and young people for the past 9 years and is supported by the Corporate Parenting Board and CPOG. The Big Survey has been exceedingly successful in capturing the voices of looked after children & care leavers in Surrey and most importantly making positive changes and differences to the lives of our young people.

Children and Young People's Haven Hours Review

The Children and Young People's Haven is a mental health crisis drop in for young people aged 10 - 18 years. Young people can drop in and get support from a Mental Health Practitioner, a Youth Worker or an Advocate. For more information, visit the CYP Havens website.

We asked young people to answer a short survey to help us look at new potential hours for the CYP Havens. We wanted to know what hours the CYP Haven should open and close on week days and weekends. We also wanted to know what days young people would like their local CYP Haven to be open.
