Registering an adult disability or impairment in Surrey

Surrey Disability Register helps anyone with a disability or impairment living within Surrey to be independent and to lead a fulfilling life. It opens doors to new networks and contacts, through our quarterly newsletter we keep you informed and up-to-date with what is happening.

Benefits for you

When you register we will send you information relevant to your disability or impairment and you can work together with us and others to find ways to influence what is important to you.

Upon registration, you will receive an identity card (ID) which is written proof that you have registered a disability with Surrey County Council. The ID card can be used:

  • if you are entitled to a disabled person's bus pass
  • where places like leisure centres, cinemas, theme parks offer discounts for disabled customers
  • to obtain VAT savings on equipment and services purchased in connection with your disability.

In addition, for your peace of mind we work with Surrey Fire and Rescue and if needed will share your address for emergency purposes, such as emergency evacuation, flooding, fire etc. This means that they will have a record that shows any address where someone may need additional assistance in an emergency.

The Register is about empowering you through information and contacts. You do not need to be a member to obtain services, apply for welfare benefits, or to ask for an assessment of your social care needs.

However, to help plan services we will use the information in The Register to help us identify what people find most helpful, but we will always make sure your personal details remain anonymous.

Patient Passport

The Patient Passport (PDF) is a document which you complete, that assists healthcare staff to provide the right help for you during your stay in hospital.

This passport has been designed so that healthcare professionals are made aware of a patient’s needs and requirements caused by a long term neurological condition so they can provide you with the best possible healthcare whenever you visit hospital.

When you have completed and printed out your passport, take it with you whenever you visit hospital and show it to the doctors, nurses and any other healthcare professionals you come into contact with. If you are admitted to hospital, ask the nurses to keep your passport with your notes at the bottom of your bed.
The passport is not expected to hold all the available information about the person, however please complete it as fully as possible. It should however contain enough detail to enable staff within the health service to understand your everyday needs. The Patient Passport makes a connection with those who know you least (staff in hospitals) and with those people who know you best (carers/ parents /care co-coordinators /specialist nurses/ community nurses/therapists etc.)
