The Adult Social Care service at Surrey County Council focuses its support on people with social care needs and carers to help them remain safe, well and independent in their communities. The service ensures people in most need of support are prioritised, particularly:

  • people with long-term and complex needs that affect their daily life;
  • people recovering from an accident or illness who need help to manage daily living tasks, or build confidence over a short period of time;
  • people recovering from a mental illness; and
  • carers of all ages.

Residents with care and support needs can be referred and supported via the following means:


Professional referrals to adult social care

  1. Direct residents to complete a quick online self-assessment to find out if they might be eligible for social care support and if they will need to pay. Available at Adult Social Care Online will:
    • indicate if they are likely to qualify for social care support from Surrey County Council;
    • signpost them to local services that are relevant to their needs; and
    • help them decide whether to continue with a more detailed assessment.
  2. For general social care information and advice, as well as out of hours emergencies, contact adult social care.
  3. Report a concern about the safety of an adult with care and support needs via the Surrey Multi-Agency Safeguarding Hub (MASH).
  4. Refer a resident to a local social care team via the new adult social care professional referral form (DOC).
  5. Refer a carer to information and support via the GP carers' prescription.

Community support is available across each of the 11 localities in Surrey.

Local social prescribing services

Social prescribing is a way for professionals to link patients in primary care with sources of support within the community. It provides a non-medical referral option that can operate alongside existing treatments to improve health and wellbeing.
