Health and social care services nationally are increasingly working together to provide better support for residents' care needs.
Partnerships – known as sustainability and transformation partnerships (STPs) and integrated care systems (ICSs) – are aiming to:
- improve quality and developing new models of care;
- improve health and wellbeing of local people; and
- improve efficiency of services.
The partnerships are made up of local authorities, clinical commissioning groups, health service providers and a range of local partners.
They set out how health and social care services need to change over the five year period from 2016-2021 to achieve the right care, and cover 44 geographical areas across all of England.

- Frimley Health and Care – covering the Surrey Heath, North East Hampshire and Farnham Clinical Commissioning Group areas (plus some areas outside the county).
- Surrey Heartlands Health and Care Partnership – covering the Guildford and Waverley, North West Surrey, Surrey Downs, and East Surrey Clinical Commissioning Group areas.
Emotional Wellbeing and Health
Surrey County Council and the Clinical Commissioning Groups (CCGs) in Surrey are committed to supporting children and young people to have the best start in life. A fundamental element of having this best start is their emotional wellbeing and mental health (EWMH). Both nationally and in Surrey, there is a consensus that we need to change the way we think about and deliver Emotional Wellbeing and Mental Health (EWMH) services for children.
