A new Surrey online Equipment Self Assessment Tool is available from Adult Social Care, designed to help people purchase equipment with confidence and live more independently.
The online assessment tool is easy to use and designed for people who find it difficult to do everyday things like:
- Get in and out of the bath
- Get up and down from low places, like a chair
- Get around easily.
If you want to take control of your own health or personal situation, have equipment needs but are not eligible for social care services, or would prefer to choose and pay for your own equipment, then you can benefit from our online assessment.

What is the Equipment Self Assessment Tool?
It is a web-based tool that asks a series of questions related to a selected area of need e.g the bathroom, or turning the taps on and off, which results in a list of equipment suggestions that specifically meets those needs.
Who is this service for?
- Anyone who has encountered issues with managing daily living tasks and feels that equipment would enable them to manage more effectively
- People who can afford or choose to pay for their own equipment
- People who do not meet the eligibility criteria for help from Adult Social Care; you do not need to have eligible social care needs to undertake this assessment online
- However, if you believe that you may have eligible social care needs then please refer to How to access adult social care services for further information about eligibility, or contact the Adult Social Care helpline who will advise you.
What are the benefits of this service?
- It is an easy to use system that is clinically supported by health professionals
- People can take some control of their own life by ordering equipment which they know will be appropriate
- At the end of the assessment, it provides people with a list of suggestions matched to their individual needs and measurements.
Is there a cost for this service?
- There is no cost or charge to use the online self assessment
- At the end of the assessment you will be provided with equipment suggestions, prices and where these can be purchased.
What if I do not have access to a computer?
You can go into a Surrey library, or get support at The Surrey Hubs based in Epsom, Redhill or Woking to complete the assessment, or for information, advice, advocacy and other services about care and support.
What information do I need before I begin my assessment?
- You will need to register your details, including an email address. If you do not have an email address already set up, you can set one up free at www.hotmail.co.uk, www.yahoo.co.uk or www.google.co.uk
- You will need to know your height (in feet and inches, or centimetres) and weight (in stones and pounds, or kilograms)
- You may need measurements of the area you need help with, for example; your bath (width and length), toilet (height from floor to bowl) or chair (height of seat). These measurements can be in feet and inches or centimetres.
How long does the assessment take to complete?
- Depending on your needs and speed of using a computer, the assessment can take between 30 minutes to one hour to complete.
- At the end of the assessment, you will be provided with equipment suggestions and where these can be purchased.
What if I have problems completing my online self assessment?
- If you find that you are having problems, contact the Adult Social Care helpline and they will be able to answer any questions you may have and assist you in completing your online self assessment;

Some examples of equipment that might be suggested are:
- Grab rails designed to provide support and leverage at the top and bottom of staircases
- Raised toilet seats, designed to help you to get on and off your toilet easily and without assistance
- Adjustable perching stools giving you balance and stability while washing up, ironing or cooking
- Telephone amplified ringers and door triggers designed to help those individuals who find it hard to hear the telephone or door bell
- Walking frames giving you increased balance, stability and confidence to move around your home with ease
- Bath lifts designed to offer safety, comfort and freedom when bathing.
What happens after I purchase my equipment?
- Any equipment purchased by you a result of this online self-assessment will not be followed up by a social care team.
- Normal manufacturers warranties and after sales service should apply.
