Home based care

Previously referred to as domiciliary care, home based care services are services provided to you in your own home. The care is provided by community care assistants, who work for us.
To be eligible for care in your own home, you will need to have completed a community care assessment, or a carers assessment.

How do I apply for a community care assessment

You, your doctor, health worker or relative can request an assessment by telephoning us on 0300 200 1005.

How do you assess my eligibility for care services?

We will assess your needs to find out what services you are eligible for and will discuss this with you after the assessment.
If we find that you are eligible for services, we will draw up a care plan, which defines the type and the level of care you require.
You will receive a copy of the care plan that has to be agreed by all parties, including you, a carer or Advocate on your behalf and the Care Manager.

How can I let you know what I think of your services?

The aim of our service is to provide quality care to our service users whilst meeting their needs and wishes. To enable us to maintain this we rely on feedback from yourself and your family and/or your carers. You can use the complaints or compliments form which is in the assessment folder you were given by your community care assistant.
  • If your community care assistant is doing a particularly good job please let us know, we like to give praise where it is due.
  • If you have a positive suggestion as to how we can improve our existing services, we would like to hear about it so we can adapt and improve.
  • If you are unhappy about something, please let us know. It is very important that you are aware that you can tell us anything you are unhappy about. To do so, you can in the first instance talk to your community care assistant or contact your team leader, who made your assessment. You can also contact Home Based Care on 0300 200 1005.
